Meeting 2 November 2011
In this pack you will find a welcome booklet with all the basic
information for the course. On the last page is a calendar for the
course.
The pack also includes the following:
- Confirmation covenant
- Confidential Information form
- Parental consent for the course
- Retreat Weekend Form
These must be returned on Wednesday 2 November with the candidate.
Confirmation Covenant
We request that the code of conduct on page 5 be read carefully
before the Covenant is signed. The covenant is a three way pact between
the candidate, the parents and the parish. Each party promises to play
their part. Fr. John will sign on behalf of the parish when the forms
are returned.
Confidential Information form
Please can you also complete this form – we need to know if any candidates have any medical conditions.
It would be helpful to have some information of what do in in an emergency.
Consent form
This must be signed and returned. It is important that we have contact information in case of emergency.
Retreat reservation form
If the candidate wishes to come on the retreat, this must be completed and returned with a deposit of £25.
